Applications and instructions for the four types of facilities/services the Adult Care Licensure Section either issues a license for or receives registration of: adult care homes (7 or more beds); family care homes (2-6 beds); overnight respite services (within a certified adult day program or adult day health program); or multi-unit assisted housing with services.

Facility Closure Information
The procedures listed below outline the required steps for the voluntary closure of an adult care home or family care home in North Carolina.
Procedures:
Pursuant to 10A NCAC 13F and 10A NCAC 13G .0211, if a licensee plans to close a home, the licensee shall provide written notification of the planned closing to the Division of Health Service Regulation, the county department of social services and the residents or their responsible persons at least 30 days prior to the planned closing. Written notification shall include the date of closing and plans to assist residents and families with relocation.
Closing and Relinquishing the License
If the facility is closing and the licensee is relinquishing the license (the facility will no longer operate) please complete the following steps:
- Download, complete, and email the FACILITY 30 DAY NOTICE OF CLOSURE FORM to the Adult Care Licensure Section at [email protected].

Submit a License Relinquishing Letter. The letter shall include the following information:
- Intent to close the facility
- Intent to relinquish the facility license
- Date of Closure
- Email the relinquishing letter to ACLS at [email protected].
Closing but Not Relinquishing the License
If the facility is closing and the licensee is not relinquishing the license (such as temporary closure where the license remains active), please download, complete, and email the FACILITY 30 DAY NOTICE OF CLOSURE FORM (link here to form) to the Adult Care Licensure Section at [email protected].
Applicable Rules
Please note the following Rules that apply to facility closures.
Adult Care Home:
- Pursuant to 10A NCAC 13F .0302(d), any existing licensed facility that is closed or vacant for more than one year shall meet all requirements of a new facility.
- Pursuant to 10A NCAC 13F .0302(a), all new construction, additions and renovations to existing buildings shall have drawings submitted by the owner or his appointed representative to the Division of Health Service Regulation Construction Section for review and approval prior to commencement of the work.
- Pursuant to 10A NCAC 13F .0302(f), the facility shall have current sanitation and fire and building safety inspection reports which shall be maintained in the home and available for review. This applies to facilities when the license is maintained and not being relinquished.
Family Care Home:
- Pursuant to 10A NCAC 13G .0302(d), any existing licensed home when the license is terminated for more than 60 days shall meet all requirements of a new home prior to being relicensed.
- Pursuant to 10A NCAC 13G .0302(e), any existing licensed home that plans to have new construction, remodeling or physical changes done to the facility shall have drawings submitted by the owner or his appointed representative to the Division of Health Service Regulation Construction Section for review and approval prior to commencement of the work.
- Pursuant to 10A NCAC 13G .0302(n), the facility shall have current sanitation and fire and building safety inspection reports which shall be maintained in the home and available for review. This applies to facilities when the license is maintained and not being relinquished.
