Progress Report Form

After a certificate of need is issued, the certificate holder is required to periodically report what progress has been made on the development of the project. The certificate holder completes the Progress Report Form and submits it to the Certificate of Need (CON) Section. The CON Section reviews the completed Progress Report Form to determine if the certificate holder is making a good faith effort to develop the project consistent with the representations made in the certificate of need application and any conditions imposed on the certificate. 

The CON Section may withdraw the certificate of need for the following reasons:
