  3. CON Application Forms:
    1. There are two different application forms: One for dialysis services and one for all other types of services.
    2. A copy of the application forms can be obtained by sending an email request to [email protected] . Please provide the information required by 10A NCAC 14C .0203(c) in your request.
  4. Reports: Reports are posted in a PDF format on the Agency's website. If the reports are for activity during July, they will be posted on the web in mid-August.
  5. Decisions and Findings: Decision letters and findings are posted on a monthly basis in a PDF format on the Agency's website. They should be available on the website on or about the 15th of the following month.
  6. No Reviews and Exemptions: No review and exemption letters are posted on a monthly basis in a PDF format on the Agency’s website. They should be available on the website on or about the 15th of the following month.
  7. Written Comments:
    1. Comments should be submitted as an attachment to an email.
    2. Comments must be sent to the following email address [email protected] and to the analyst assigned to the review.
    3. Comments must be received no later than 5:00 p.m. on the date the comments are due.
    4. Written comments are posted on a monthly basis in a PDF format on the Agency's website. The comments should be available on the website on or about the 3rd workday following the last day of the comment period. If for some reason they are not available, the PDF can be obtained by sending an email to [email protected].
  8. Certificates: Certificates are posted in a PDF format on the Agency's website on a monthly basis beginning with certificates issued during January 2013. The certificates are available in one PDF as part of the Certificates Issued Report.
  9. Progress Report Form: The Progress Report Form is available on the Agency's website in either Word or as a PDF.
