Revised Certificate of Need Application for Medicare-certified Home Health Agencies

The certificate of need application has been revised effective January 5, 2011. The revised certificate of need application must be used for all proposals to develop a home health agency. This application replaces all previous Home Health Agency applications and must be used for all reviews beginning on or after April 1, 2011.

Prospective applicants are encouraged to contact the Certificate of Need Section to schedule a pre-application conference with the project analyst who is assigned to the county for which you are applying. The conference should be scheduled well in advance of the date the application is to be filed. Please do not hesitate to call the project analyst if you have any questions regarding how to complete the revised application.
