  3. CON Application Forms:
    1. There are two different application forms, each with 3 components:
      1. Dialysis Services
      2. All other types of services
    2. A copy of the application forms can also be obtained by sending an email request to [email protected] . Please provide the information required by 10A NCAC 14C .0203(c) in your request.
    3. Please note: an application for a CON may not be needed. Please check with the Project Analyst for the county in which you propose to develop the health service. A list of counties by Project Analyst and the Project Analysts’ email addresses are included in the County Assignments Chart on the Staff page.
  4. Reports: Reports are posted in a PDF format on the Agency's website. If the reports are for activity during July, they will be posted on the web in mid-August.
  5. Decisions and Findings: Decision letters and findings are posted on a monthly basis in a PDF format on the Agency's website. They should be available on the website on or about the 15th of the following month.
  6. No Reviews and Exemptions: No review and exemption letters are posted on a monthly basis in a PDF format on the Agency’s website. They should be available on the website on or about the 15th of the following month.
  7. Written Comments:
    1. Comments should be submitted as an attachment to an email.
    2. Comments must be sent to the following email address [email protected] and to the analyst assigned to the review.
    3. Comments must be received no later than 5:00 p.m. on the date the comments are due.
    4. Written comments are posted on a monthly basis in a PDF format on the Agency's website. The comments should be available on the website on or about the 3rd workday following the last day of the comment period. If for some reason they are not available, the PDF can be obtained by sending an email to [email protected].
  8. Certificates: Certificates are posted in a PDF format on the Agency's website on a monthly basis beginning with certificates issued during January 2013. The certificates are available in one PDF as part of the Certificates Issued Report. They should be available on the website on or about the 15th of the following month.
  9. Progress Report Form: The Progress Report Form is available on the Agency's website in either Word or as a PDF.
